Children are considered immunized against measles, mumps, & rubella (MMR) when they have received 2 doses of MMR or 1 dose of MMR and 1 dose of MMRV-includes varicella (chickenpox): First dose soon after first birthday; Second dose between ages 4-6.

Immunization card lost? If you received your immunizations in our office or you transferred to our office and previous medical information was transferred to our office, we can prepare a duplicate immunization record. $30 is charged to prepare a duplicate record.
